All student record changes take place in your student portal.  

Name Change

To update your name, you will need to request a Student Information Update Form through your student portal, under MyForm section. When submitting the form, please make sure to include a valid proof of your name change such as a marriage license or a valid driver's license.

Date of Birth

To submit your date of birth, you will need to request a Student Information Update Form through your student portal, under MyForm section. When submitting the form, please make sure to include a valid proof of your date of birth.

Address

To update your home address, you will need to request a Student Information Update Form through your student portal, under MyForm section.

Social Security

To submit your Social Security update, you will need to request a Student Information Update Form through your student portal, under MyForm section. When submitting the form, please make sure to include a valid proof of your SSN.

Electronic Degree Verifications

The University of Texas Permian Basin has authorized the National Student Clearinghouse, to provide verification for UT Permian Basin students, alumni, and third-party vendors.

To request an electronic verification:

• Go to the National Student Clearinghouse
• Click on Verify Now
• Create an account
• Login to the account
• Verify your login via multi-factor authentication
• Fill in the requested personal information, and click Submit
• Fill in payment information

Within minutes after placing an order, the National Student Clearinghouse will present you with a verification certificate or notification of pending research of academic record. This verification page also includes a link to a PDF version of your verification certificate containing the same information, which you can print and/or save.

Please Note: The National Student Clearinghouse cannot provide enrollment verifications for current students who have requested that their directory information be kept confidential. If your directory information is confidential, you must request enrollment verification through the Office of the Registrar directly at registrar@utpb.edu.

Enrollment Verification Letters

Enrollment verification letters can be requested through your student portal. Look for Academic Records on the side menu and then go to Request Enrollment Verification.

Hard-Copy Verifications

You can request a hard-copy verification by visiting the Office of the Registrar.
